Dietitian - Bourke

NSW Outback Division of General Practice (ODGP) is commissioned by the Western NSW Primary Health Network (WPHN) to manage a chronic disease service ‘Keeping Health Puura Manti Program (KHPMP)’.  The focus of the KHPMP is to provide services where there is no alternative funding or services available for chronic disease prevention and management.
We are seeking to appoint a suitable Dietitian with chronic disease management experience (or an interest in this area) to a position based in Bourke. Please note – motivated new graduates will be considered for the role and will be supported with Clinical Supervision. The successful candidate will work with the KHPMP enrolled general practices and Aboriginal Community Controlled Health Organisations (ACCHOs) within Bourke and Surrounding LGA’s.

Bourke Pathways Care Navigation Program

The NSW Outback Division of General Practice (ODGP) is a health service provider working to improve the health of people living in the outback shires of Cobar, Bourke, Brewarrina, and Walgett. ODGP manages a variety of health contracts focusing on early intervention, prevention of disease, chronic disease management, mental health care and health promotion programs. ODGP currently has a vacancy for 2 fulltime positions for a Care Navigator and a Care Navigator – Team Leader operating under a new Bourke Pathways project and is looking for enthusiastic and community minded people to join the organisation and contribute to our efforts to improve the health and wellbeing of people living in north-western NSW.

The Bourke Pathways Care Navigation Program is a pilot program funded by nib and is designed to contribute to sustained improvement in health outcomes for eligible clients through the facilitation of access to health, community, and social services as well as increased health literacy.

BOURKE PATHWAYS CARE NAVIGATOR - TEAM LEADER

1 Full Time Position – Bourke NSW

 

This position will lead the implementation of the Bourke Pathways Care Navigation Program. The successful candidate will be responsible for care navigation, supervision and leadership of the navigation team, reporting, program oversight, community liaison, project promotion, meetings with stakeholders and funding partners and the implementation of training plans.

A salary package of up to $130 000 is available with a starting hourly rate of $50.00 plus superannuation, 5 weeks annual leave, leave loading and an additional salary sacrificing benefit up to the amount of $16 050 per annum.

 

BOURKE PATHWAYS CARE NAVIGATOR

1 Full Time Position – Bourke NSW

 

This position will provide administrative support to the Team Leader and support the implementation of the Bourke Pathways Care Navigation Program. The successful candidate will be responsible for care navigation, reporting and administrative functions, database reporting, community liaison and project promotion.

A salary package of up to $100 000 is available with a starting hourly rate of $40.00 plus superannuation, 5 weeks annual leave, leave loading and an additional salary sacrificing benefit up to the amount of $16 050 per annum.

3 Mental Health Nurse Vacancies

Bourke, Cobar and Lightning Ridge NSW

The NSW Outback Division of General Practice (NSW ODGP) is a health service provider working to improve the health of people living in the outback shires of Cobar, Bourke, Brewarrina, and Walgett. NSW ODGP currently has the following full time Mental Health Nurse vacancies in Bourke and Lightning Ridge and is looking for enthusiastic professionals to join the organisation and contribute to our efforts to improve the health and wellbeing of people living in north-western NSW.  The NSW ODGP manages a variety of health contracts focusing on early intervention and prevention of disease, chronic disease management, mental health care, healthy lifestyle programs and health promotion.

The Mental Health Nurse will work within the ODGP MHNIP program based upon an innovative, a successful model of care and collaborative service framework involving Local Health District Mental Health Services, General Practice and Aboriginal Health Community controlled services. The MHNIP positions will be working directly with General Practice led multidisciplinary teams in both mainstream GP and Aboriginal Medical Service settings as well as undertaking brief clinical placements with the Local Health District Mental Health services to strengthen the Stepped Model of Care.

Highly attractive salary packages (based on CNS rate - $94,000 to $102,000 plus leave loading and superannuation) will be paid according to qualifications and experience and will be in line with the relevant modern awards. In addition, As a public benevolent health organisation, NSW Outback Division of General Practice Ltd employees are entitled to an attractive salary packaging component or remuneration.  Applicants may also be eligible for substantial financial support for relocation and professional development under the NSWRDN administered Rural Health Professionals Program (RHPP).
