Community and Social Care Coordinator
The NSW Outback Division of General Practice (ODGP) is a health service provider that works to improve the health of people living in Western New South Wales. The ODGP manages a variety of health contracts focusing on early intervention and prevention of chronic diseases, chronic disease management, care coordination, lifestyle modification and health promotion programs.
The ODGP also operates the Cobar Primary Health Care Centre (CPHCC) which is a highly functioning General Practices consisting of 3 Doctors, Practice Nurses, Mental Health Nurse, Allied Health Professionals and other visiting health services. The CPHCC is a funded Integrated Care site by the Western NSW Local Health District with the Integrated Care program in Cobar initially focused on improving the overall health and well-being of people dealing with chronic and complex care needs.
The ODGP has an existing vacancy for the position of Community and Social Care Coordinator which will be based at the Cobar PHCC.The position will assist patients, carers and families with a chronic or complex condition through psychosocial support and advocacy, counselling, information provision and access to community resources to better manage their health. The Community and Social Care Coordinator will function as a member of the GP Led Multidisciplinary team and have a pivotal role where social complexity exists.
The position is contracted until the 30th of June 2018, with possible extension of the contract until the 30th of June 2019. A highly attractive salary package is offered based on skills and experience under the Health Professional and Support Services Award.
